Gatefold sleeve with 2-page booklet glued inside.
Sides are auto-coupled, sides A (1) and D (4) on LP1, sides B (2) and C (3) on LP2 
Terre Haute pressing, like [r=19890211, but this version has beige labels, similar to [r=1157120.
